# Can HRT prevent or worsen oestrogen dominance?

**Oestrogen Dominance**

Oestrogen dominance is often described as an imbalance between oestrogen and progesterone levels in the body. During perimenopause, women often experience fluctuations in hormone levels, which can lead to periods where oestrogen levels are disproportionately high relative to progesterone. This hormonal imbalance can cause a range of symptoms, including weight gain, mood swings, breast tenderness, and menstrual irregularities.

**Oestrogen Dominance and Hormone Replacement Therapy (HRT)**

The concept of oestrogen dominance during perimenopause, particularly in the context of hormone replacement therapy (HRT), remains controversial. While some evidence suggests that HRT can lead to symptoms associated with oestrogen dominance, the extent and consistency of this evidence vary.

HRT is commonly used to alleviate the symptoms of menopause by supplementing the body’s declining hormone levels. However, if not properly balanced, this therapy can sometimes exacerbate the symptoms of oestrogen dominance.

Careful management and individualised medical treatment plans are crucial to mitigate these potential effects and balance hormone levels effectively.

**NICE guidelines**

The NICE guidelines emphasise careful management of HRT to mitigate potential adverse effects and balance hormone levels effectively. Key points include:
1. Individualised Treatment: HRT should be tailored to the individual's needs, taking into account their symptoms, health risks, and personal preferences. This approach aims to ensure that hormone levels are balanced and to minimise the risk of oestrogen dominance.
2. Monitoring and Adjustments: Women starting HRT should have a review at 3 months to assess the treatment's effectiveness and side effects. This helps to ensure that hormone levels are appropriately balanced and to adjust dosages if necessary.
3. Benefits and Risks: HRT can alleviate menopausal symptoms and improve quality of life. However, it is important to monitor for potential side effects, particularly with oral preparations.
